It is essential that you are medically fit to dive. We will therefore ask you to complete a medical health questionnaire before you start diving with the club.

If you have Adobe Acrobat Reader software installed on your computer, you can click the Acrobat icon (left) to download a copy of the questionaire.

If you have a medical condition and are unable to answer 'YES' to all of the questions, it does not necessarily mean that you will be unable to dive. But we will ask you to visit a doctor for a full medical examination.

There are two doctors that we have arrangements with who will carry out the necessary medical if required:
